Umbrella Task for ItemTreePanel and ItemFormPanel issues

Description

Functional descriptions:
Evaluate if gradebook2 handles all the grade item edit scenarios correctly as well as only show edit fields that are relevant to the select gradebook type.

Items created within Categories not included in grade do not allow persistent % of Category

Rclick>Add new grde item or File>New item, after New item frame opens, click on a category and the New item heading remains in the frame, but the function is edit category.

In Weighted Categories/Points mode GB2, %Category value for items in an un-included category disappear under certain conditions.

CCFIT: During use of add item dialogue to enter items in multiple categories, changing from a category with equal weighting specified to a category with equal weighting not set, the item weight entry field does not appear.

"%-of-category" field disappears from "new-item" dialog at times when it shouldn't

CCFIT: Drop lowest problem in non equally weighted categories mode

After gradebook reset, Rclick in the empty space below the defined list of categories/items in the grade structure area, the Rclick menu shows all possible choices, but none of them work.

GB setup panel (gb or category or item), changes to displayed values are not cleared from buffers when cancel button chosen. Causes popup when any other entry is selected for settings display.

For a Cat GB w/ a drop lowest cat, warn users if the item points are not all equal
