Should not be possible to hard delete assesment with answers

We have had cases of quiz authors "accidentally" deleting a published assessment that students have answered. This can cause serious support load.

Suggested solution:

• it should not be possible for a user to delete a published assessment with answers from the database.

• rather they should be flagged as deleted which suppresses display in the UI, but makes restoration trivial.
