samigo: With multiple single selection (MC & Survey) questions on a page, ".undefined" displays in answer choices.

Description

If you create an assessment with more than one Multiple Choice, Single Selection* or *Survey* question on a page, the answer choices for these questions display with .undefined* (sometimes the word *undefined* is repeated several times, e.g. *undefinedundefinedundefined*).

The additional undefinedundefined is also read by a screen reader, as shown with ChromeVox in this recording: https://www.screencast.com/t/AbrxFEshnlE6.  The issue makes questions extremely difficult to answer, if the student can handle answering them at all with all the confusing "undefined" repetitions.

Need to note that there is a problem with the multiple choice questions when viewing them on the page in the zip file, (all the answer choices are moved down from the selectors). I tried to reproduce by creating mc questions, exporting and importing, but they were all fine. I emailed Tiffany in case this was a result of the quiz being created while there were problems with the mc questions.
