Error correction flaw with Tests & Quizzes.

Description

Finding that green arrows showing correct answers show up on incorrectly answered questions after first viewing of student's work. See attached.

There is a use case where this fix is not working, I have attached a patch in SAM-2253.

I fixed the case in trunk r131905 and 2.9.x r131906.

It was committed with mixed case: SAM_ITEMGRADING_t

Passed testing on RC05 Sakai 2.9.3-rc05 (Kernel 1.3.3-rc05)

I've retested the Test Plan, plus a little more. I had three students take the FITB. One student got all 5 answers wrong (original Test plan), one got all question right, and one got 3 out of 5 . It was worth 10 points, so the scores were 0, 10, and 6 respectively. I checked that the feedback was correct (number of red x's and green check marks on each one) , adjusted the scores as instructor, made sure the scores changed and the red x's and green check marks still reflected the answers, and they did.

I moved the conversion script to 2.9.2 - 2.9.3 in r128523.
