Date pickers disappear when using Extended Delivery option on published assessment

Description

To reproduce:

1) Create an exam and add an Time Limit/Delivery Date Exception
2) Save settings and publish
3) Go back to add additional exceptions. You can't because the date pickers have disappeared in the Time Limit/Delivery Date Exception section.

It seems to me that this may not be a problem for working copies, only published.
