Assignments entity provider lacks several data elements

Description

There are still several assignments fields missing from the existing entity provider that we'd like to include:

1) Grade scale
2) Supplement Items text (Modal Answer, Private Note, All Purpose note)
3) Student submission (type)
4) Allow resubmission flag

Dashboard build has been fixed. Assigning this back to Reggie for merging into CTools release.

Tested the entity feed and confirmed all the requested fields are there.

There are many other attributes in the Supplement Item and resubmission settings. However, since Beth clearly indicated the output is for "supplement text" and "resubmission flag", let just limit the export to those for now.
