Attempted deletion of a duplicate named Item in a Category generates a Request Failed warning.

Description

1) Create situation of duplicate named Items within a Category per Bug GRBK-189.

2) Begin Delete action on one of the duplicate named Items.

3) As expected, warning in red appears: "Deleting this item will remove it from the grade book. The grade records for this item will no longer be used to calculate course grades and will no longer be accessible to the students even if the item was released prior to being deleted."

4) Attempting to actually delete the Item generates a Request Failed: "Failed to update item: There is already an existing item called "<name of Item>" in this category. Please enter a different name for the grade item."

5) Editing the Item to change the name allows the name to be changed, but the Deletion action previously disallowed now automatically occurs after refresh of Gradebook 2 and the renamed Item is summarily deleted.

This did not take external tools into account... specifically, external tools can create two separate assignments with the same name but different case: "essay and "EsSaY" ..... edu-services treats them as separate and gb2 treats them as the same.

This ticket was never addressed directly since the assumption was that the situation would not be possible, attempting to delete one still fails.

So this still needs to be fixed

We're going to resolve this problem by preventing the duplicates in the first place. A workaround would be to rename the items before deleting them.
