Update text on Evaluations displayed to Students:

Current text: "Evaluatee/Instructor Items: <Instructor Name>"
Proposed text: "Complete below if your instructor is: <Instructor Name>"

Text is found in evaluation/tool/src/java/org/sakaiproject/evaluation/tool/bundle/messages.properties

Probably the takeeval.instructor.questions.header property.

Changing the background color fix is not implemented. since Evaluations can't be created locally and it is not easy to fix in that case. This was discussed with Lisa
