Duplicate settings appear when clicking 'Create'

Description

When creating a new assessment in Tests & Quizzes, the radio buttons are duplicated in you click Create without clicking one of the settings.

Steps to reproduce
1. Open Tests & Quizzes
2. Do not choose 'Create using assessment builder' or 'Create using markup'
3. Click Create.

Appears to be fixed in latest version of Sakai 11.

Tested on Marist Qa server. http://qa01-sakai.marist.edu:8080/portal/
