Determining what are duplicate item names should be case-insensitive

Description

Consistently handel duplicate item name handling :

  • ItemFormPanel

  • Import

  • External Tools (Assignments, Samigo, Forums)

Specification:
Names are not cases sensitive. e.g. "Finals" and "finals" are not allowed

This issue may be moot since the external assessment service finds collision disregarding case... so only users of a tool service could create the condition: gradebook 1 or gradebook 2 are the only current possibilities AFAICS

I think we need to consider gradebook2 taking control of the duplicate name resolution: identifying dups on load and providing a radio button list of choices for each duplication... and, once all the names are resolved by deleting or renaming, the gb would load.
