The javascript method resetMultipleChoiceAnswers in show-page.js corresponds to the Cancel button for the Edit Question dialog. It’s missing a clean-up step that was implemented in Sakai 20 and earlier, prior to UI layout improvements implemented with .
I think I have a fix and hope to make a PR with it soon.

I have confirmed that this bug is active on the nightly server for 23.x and 22.x, in addition to 21.x.
If you save after the second edit, do the new options get created? Or is it just a visual issue on the edit panel?
If you click Update Item during the second edit, the new options appearing in the dialog are then added as new options for the question.
The Add Content >> Add Content >> Question >> MCQ incorrect answer options appear to be repeated after being posted.
