Testing this Jira I found that the item appears twice in the confirmation page, fixed in SAK-42838.

In Syllabus tool, create a new item.
Add attachment to that item
Select Edit tab
Select Edit details
Select Remove under the attachment
A message appears asking to verify if you would like to remove the attachment and the attachment is listed twice.
Press Delete
The attachment is still included in the syllabus item
The only way to remove is to press the trash can icon next to the attachment on the main syllabus screen.
