Tested the issue in the following server:https://trunk-mysql.nightly.sakaiproject.org/portal/site and browser :Google Chrome. It works fine. Please find attached the screenshot.

Tested the issue and it works good. The tabs are style correctly and the questions tab is present. Please find attached the screenshot.

If you access a student's submission by clicking a link in the User Activity Report, the links at the top of the page are styled incorrectly and Questions link is missing.
Testing Steps:
In a site with at least one student, create a quiz.
As a student, take the quiz.
Log in as instructor and go to Tests & Quizzes > User Activity Report
Select student.
Select submission.
If Issue Present: The tabs, Submission Status, Total Scores, etc. are styled incorrectly (as links - inconsistent with views in other Scores screens) and the Questions tab is missing completely.
To see correct display, go to Published Copies > Scores > click student's name.
