The formatting is off on this page, with "Required items marked with a" appearing as a blue info box and "Choose File" appearing as a dark grey box (not the browser default).
I notice that the text says "Complete the form, then choose the appropriate button at the bottom."
This doesn't really make sense to me. It seems meaningless (no offense intended). I think it should be removed, though that would be a separate Jira. Thoughts?

This is the new style in 12.x Removed info box and removed some spaces.
Click "Add."
