Formatting for All Grades & Course Grades is completely messed up

Description

If you look at the All Grades & Course Grades tabs, the formatting is all messed up. Everything seems to be pushed to the extreme. The categories are pushed all the way to the right and the student/grade listing is pushed to the bottom.

The change seems to be in

inlineForm fieldset{display:inline-block} in tool.css but I can't see where that was incorporated in 12.

This looks fine in Sakai 12, maybe missing a merge somewhere.

Checked it as of 4/8/2016 and it doesn't seem to be happening anymore.
