Allow LTI tools to go fullscreen

As described at:

http://stackoverflow.com/questions/14151899/when-using-video-js-why-doesnt-the-full-screen-button-work-inside-an-iframe

some HTML pages are unable to go full-screen (typically video playback) if the iframe does not include the allowfullscreen attribute.

This should be set by default for LTI iframes, or if that's not desireable, at least an option per-tool or per-placement.

There are several other places these need to be added to iframes.  In lessonbuilder and in the basiclti-portlet tool.

We've been running this patch in 10.x for a while with no ill effects.

Yes, we ran into the problem with LTI but would also be an issue for the other iframe tools.

Great idea. I assume you want this for the iframe and sakai.web.168 tools too.
