Drop box / Always send email notifications does not send email

Description

Drop box:

If the instructor sets the option for "Allow email notifications to be sent" and checks the email checkbox when uploading a file, an email will be sent to the student letting them know the file has been uploaded.

If the instructor sets the option for "Always send email notifications", no email is sent out when the file is uploaded to the drop box.

I tested on nightly and observed the correct email was sent (in logs) to student when instructor uploaded new file.

I've fixed it for trunk, the algorithm to determine the notifications seems wrong, i've made some changes that seem reasonable.

I have checked that this is still happening in trunk.

In https://trunk-mysql.nightly.sakaiproject.org I have uploaded two files in two users dropboxes. One with "Send email to student" checkbox checked and other with "Always send email notification" checked in Options.

First email is logged in catalina.out, second email is not.
