Unable to create course site even as admin

Unable to create course site even as admin.

    • Am able to create project and portfolio sites though **

Server: http://qa3-us.sakaiproject.org:8086/portal/

To reproduce:
Log in as admin
Click My Workspace -> New
For Site Type, choose 'course site' (Winter 2012 is the default/only academic term)
Click Continue
Click 'Still cannot find your course/section'
Fill out Subject, Course and Section
Enter admin for Authorizer's username
Click Continue
On the Course Site Information page, click Continue
On the Course Site Tools page, select some tools if you wish
Click Continue
On Course Site Access page, leave the defaults: Publish Site, Limited to whom I ass manually
Click Continue
Confirm Your Course Site Setup page displays
Click Request Site
My Workspace page displays again

If you search on the course created, or use the 'More Sites', you will find the course site does not exist

After the db restore, the data is still fine on qa3-us, so someone must have changed the maintain role of the !site-template.course realm to 'maintain' instead of 'Instructor'. So, all is working as it should be.

This works OK on a fresh copy of trunk with mysql DB:

Used this SQL to insert the term:
insert into sakai_trunk.CM_ACADEMIC_SESSION_T values (13, 0, null, null, 'admin', now(), 'Winter 2012', 'Winter 2012', 'Winter 2012', '2012/01/01', '2012/04/01', 1);

Problem is Maintain role is set to "maintain" in !site.template.course.... We're looking at how this can happen

I'm wondering if there's some missing conversion? Like for a new realm or site template or something?
