include the created and last modified date on the screen and csv export

We want to include the created and last modified date on the screen and csv export. Since we are managing accounts for multiple schools and hosting accounts over several schoolyears, this additional information is very helpful for our helpdesk to figure out who is an older (existing) user and who is a new user. It would be handy at the same time to be able to get the internal userid output in the csv. The internal userid we often use as input to web services scripts and this would help us in system administration.
