Students don't see any evaluation content but the preview does

Description

I created several evaluations this morning & assigned them to a group (based on site participants from the "assign to evaluation groups" mechanism). When I look at the preview the content is there but when students look at them there is no content. This is new since yesterday at 3:00 PST when things were working fine.

I made the same mistake during testing so want to add more detail in case this happens again:
When creating a template, all questions must be assigned to the hierarchy "Top Level." Do not use a lower hierarchy, otherwise no evals will appear to user (students or instructors) and no notifications will do out.

This was related to assigning questions to hierarchy. We were constraining who could see the questions without realizing it.

We have narrowed this down to being related to the "Use view date for evaluations, otherwise just use the due date" setting in the administration interface. If it's turned on, then the questions don't appear in the evaluation (the view date was set out 2 days).
