Design bug: Need to add field sets back to border the question categories (i.e. course, instructor)

Description

Our skin doesn't have enough text size difference between the title and the content so the field set it required for visually clearer boundaries.

We've decided to push this off to our next iteration. Some concern has come up from project stakeholders about seperating instructor & course questions so we won't be using the categories for our Spring pilot.
