N/A choice on full width scale items would make more sense rendered outside of the scale

Description

N/A is not really part of the Likert-Type like continuum represented by a full width scale item, it really does not belong there. This distinction is already being made in other scale arrangements (as well as multiple choice and multiple answer layout variants). So a good case could be made that it belongs visually outside of the scale. See screenshots before.png and after.png.

The changes made to the component template in rev 74814 have been reverted in trunk and in the 1.4.x branch. They have not been reverted in the 1.3.x branch.

Instead of moving N/A option out of the row, we should separate it with a vertical bar or a change in shading/coloring.
