My Workspace page appears with Portfolio, Course and Project tabs

Click Resources to fill out the contact form to be used for the portfolio

Click Add next to 'My Workspace' folder*.* Select New Form Item

Select a Form screen appears.

Select 'Contact Form'. Click Continue

The Contact Information form fields are displayed and are ready to be filled in.

Add Item Type Select Form Item
Select Form Type Use the drop down menu to select Contact Form
