Here is the updated Add Item page per our meeting on 3 August. The "Add Additional URLs" would act like the copyright field on the Resources - Add page as Jim outlined.

Mark and I have put together a Usability Plan for everyone's review.

Usability Plan Draft

Here is an Add Item page with three additional fields: one for Full Text, Related Web Site and Custom. In the custom field label, you can select either "full text", "related web site" or "other..." If you select "other", you can write over the field label "custom" and add your own term.
